Replace the FSF boilerplate with the SPDX tag.
The SPDX tag on files *.c is incorrect, as it should use the C99
single line comment using '//'. But current checkpatch doesn't
allow C99 comments, so keep using standard C comments, by now.

Remove a use of AH_BOTTOM from configure.ac. This macro is used by
autoheader to add '#include' of some include file to the end of
config.h.in and then to config.h. OpenOCD can be built with a custom
config.h, so it's preferable to move these '#include' statement directly
in the C files that need them dropping this unneeded dependency.
It also causes problems when I want to use the gnulib library (which
comes with its own Makefile, and does not have the same include path as
the top-level Makefile).
So this change touches a lot of files, but is actually really simple. It
does not affect functionality at all.

Flash protection set on a device with MANW=1 was lost after reset.
Since #2903 the driver honored MANW bit and issued Write Page command just
for main flash write. This change adds similar technique to
samd_modify_user_row().
Minor code improvements:
samd_check_error() returns error code corresponding to error type
instead of bool.
samd_check_error() does not clear STATUS register if no error bit is set.
Eliminated double error check in call sequence samd_issue_nvmctrl_command()
folowed by samd_check_error().
Missing error code ERROR_FLASH_PROTECTED added to src/flash/common.h.

Changes from the flat namespace to heirarchical one. Instead of writing:
#include "types.h"
the following form should be used.
#include <helper/types.h>
The exception is from .c files in the same directory.
Add get_flash_bank_by_name (and get_nand_device_by_name) helpers
to retrieves struct flash_bank * (struct nand_device *) given a
driver name and an (optional) driver-specific bank index.
These are used to extend flash_command_get_bank_by_num (and
nand_command_get_device_by_num) to allow all flash (nand) commands to
reference defined banks by name, not just by number.
To avoid some code duplication, add the flash/common.[ch] files to hold
functionality common to both types driver. The first two methods are
helpers for the above routines to find a bank specified by a "name" or
"name.index" string. get_flash_name_index() finds the '.index' portion,
while flash_driver_name_matches() performs the string portion matching.
